Horizons members will attempt to reach a good portion of the 58463-area residents for input concerning the community regarding the Horizons plan to address poverty.
The members will contact you at your residence if at all possible. If no one is home, they will leave one copy of the survey and an addressed envelope. You may return it by July 15 or bring it to the July 23 community supper. If you are there, they will ask you to complete the survey and put it into a plain envelope so that they can return it to the office for tallying the information anonymously.
For each completed survey (one per resident, no age limit) you will receive a coupon entitling you to the free supper and have your name put into the door prize drawing for McClusky Merchants “dollars.” There will be several door prizes in varying amounts. You must be present to win.
